UPA
MEMBERSHIPS: All team members will need to the join the UPA* at
www.upa.org. This will give you the UPA ID#'s you need to submit your
team roster (see below). *Membership dues MUST be postmarked by April 30th.
WAIVERS: all waivers must be postmarked by april 30th. DOWNLOAD MEMBERSHIP/WAIVER FORMS HERE. Mail these to the UPA Headquarters. ROSTERING: Team rosters will need to be submitted using the UPA online system by Monday April 30th. TOURNAMENT FEES AND CHAPERONE FORMS: Your team tournament fee of $300 and your chaperone form needs to be postmarked by Monday April 30th. DOWNLOAD CHAPERIONE FORM HERE. | The day of the Championship | The authorized chaperone will be responsible for keeping the medical papers on hand. DOWNLOAD MEDICAL PAPERS HERE In the event of an injury, medical personel will need this form in order to assist an injured player. The UPA will not collect these. |
